The Independence Care System (Or ICS) was founded in 2000 as an innovating community of professionals that are dedicated to serve people who used to stay in institutions. ICS is the only “Health home” program in New York that was designed to promote the independence of people with spinal cord injuries, sclerosis, and other disabilities to limit their mobility. Few organizations in New York City contain the expert knowledge and expertise of ICS. By working with a diverse community of people with disabilities for so many years. ICS likes to focus on three essential areas: health, mobility and independence. A person with a disability who is unhealthy finds that their mobility and independence suffer. Investments in health and mobility may go to waste when someone does not have opportunities for independent activities. If a member does not have an adequately fitted mobility device, their health and independence are impacted. A member has to make all 3 of the section work in order to gain something out of the program or else their time, and money will go to waste and their health will not get better.
Health: There are specialized programs made for adults with physical disabilities. Individualized care plan focused on preventing related health conditions and improving and maintaining members’ overall health. Staff who monitor other barriers for members like including transportation, housing, education, access to primary care, and community-based resources. Advocates who monitor member eligibility, so they receive what they need, including services, equipment, and supplies.
Mobility: Accessible Wheelchair clinic. There are specialists who evaluate members who need to ensure they have the wheelchair/scooter that maximizes members’ safety, independence, and ability to participate fully in the community. They maintain backup equipment to ensure members can maintain their mobility during needed wheelchair servicing and repairs.
Independence: Allows members to participate in the community like Social and Wellness activities, peer to peer coaching to help develop independent living skills, opportunities to allow members to communicate.
